Cervicogenic Headache: The Strongest Evidence
Cervicogenic headache (CGH) — headache that originates in the cervical spine — is the headache type with the strongest evidence base for chiropractic treatment. CGH typically presents as one-sided head pain that increases with neck movement or sustained postures, often associated with neck pain and reduced cervical range of motion. Spinal manipulation and mobilization of the upper cervical spine (C1-C3) is well-supported by randomized controlled trials for reducing CGH frequency, intensity, and duration. The Canadian Chiropractic Guideline Initiative supports spinal manipulation for CGH as a first-line treatment.
Tension-Type Headache and Migraine
For tension-type headache — the most common headache type, characterized by bilateral pressure or tightening — chiropractic manipulation and soft tissue therapy show moderate evidence for reducing frequency. For migraine, chiropractic does not treat the acute migraine attack (medication remains the primary treatment) but can address cervical and musculoskeletal contributing factors that increase migraine susceptibility. A 2019 randomized trial in the European Journal of Pain found that chiropractic manipulation produced comparable reductions in migraine frequency to topiramate (a common migraine preventive medication).

Before treating headaches, your chiropractor screens for red flags that require medical assessment (thunderclap onset, new headache in someone over 50, headache with fever and stiff neck, headache with neurological symptoms). For mechanical headaches, assessment includes cervical range of motion, upper cervical palpation, nerve root testing, and postural analysis. Treatment combines cervical manipulation or mobilization, soft tissue therapy for the suboccipital muscles and upper trapezius, postural correction, and ergonomic advice for screen use and sleeping positions.
