What Your PT Will Assess
Your physiotherapist will perform a comprehensive assessment to identify the underlying drivers of your pain — whether that is joint dysfunction, muscle imbalance, nerve irritation, movement pattern faults, or psychosocial factors. Red flags (symptoms suggesting serious pathology such as fracture, infection, or malignancy) are screened at the outset. For the majority of Canadians with non-specific low back pain, reassurance and active rehabilitation — not imaging or rest — is the evidence-based starting point.
Evidence-Based Treatments for Back Pain
Current Canadian and international guidelines recommend an active, movement-based approach to low back pain. Physiotherapy treatments with strong evidence include: therapeutic exercise (targeted strengthening and motor control training); manual therapy (spinal mobilization and manipulation, particularly when combined with exercise); pain education (understanding the neuroscience of pain to reduce fear-avoidance); graded activity and graded exposure for chronic pain; and dry needling for myofascial contributions. Passive treatments like ultrasound and TENS are generally used as adjuncts, not primary interventions.
How Many Sessions Will You Need?
For acute low back pain (less than 6 weeks), most people respond well within 4 to 6 physiotherapy sessions combined with a home exercise program. Chronic low back pain (more than 12 weeks) typically requires 8 to 16 sessions, and outcomes are significantly improved when physiotherapy is combined with pain psychology support — the biopsychosocial model is now standard of care. A good physiotherapist will set measurable goals and adjust the plan based on your progress.
